when was the last time he had a water change? ammonia build up can cause that kind of behavior ( darting and acting panicky) and make them more prone to disease. I'd change the water every week ,at least, and use a dechlorinating product in the new water. (mine live on average 5 to 6 years, a pretty good record.) at this point you might wanna treat for fin rot too if his fins are tattered and he's not rubbing. your local pet store can recommend something, i'm sure. good luck!

did you ever take in mind that there might be a parasite on him and thats why hes darting around
get a magnifying glass and examine him and the fins are prabably getting stringy from fin rot thus he must be ill
treat the water with medication for external and internal illnesses medicine and if everything is fine test the water that your using
maybe the bottled or faucet water has an obsene amount of metals in it or has been thru reverse osmosis
